PERSONAL RELEASE

In consideration of my choice to participate in Terrence J. Brady's THIRD MILLENNIUM feature production JUMPCUT, I hereby irrevocably grant to the said filmmaker the right to use my image and/or voice in the above mentioned film/video. My participation in the said film/video is strictly for artistic purposes to gain experience/exposure in furthering my acting career. I have not been offered any payment for my role in the said film/video nor have I been led to believe that any payment will be made to me in the future for my participation. My participation in the said film/video is completely and utterly my own decision and I take full responsibility for any and/or all of my actions. The said filmmaker and THIRD MILLENNIUM will not be held liable for any and/or all mishaps that may occur due to my participation in the said film/video [including but not limited to] personal injury, property damage or financial hardship/loss. I give the said filmmaker the right to broadcast the said film/video in any arena [including but not limited to] film/video festivals, public/cable television, and theatrical movie houses. The said filmmaker has not made any promises [written, verbal, or implied] that I would benefit from any of the profits secured by the completed said film/video nor have any promises [written, verbal, or implied] been made that I would be offered any roles in any future productions by said filmmaker. I understand that any contracts I may have signed with [including but not limited to] agents, production companies, and union guilds and the rights those contracts guarantee me are nullified for my participation in the said film/video. I hereby irrevocably state that the said filmmaker and THIRD MILLENNIUM are not liable for any and/or all contractual obligations that I may have/have made before agreeing to participate in the said film/video. If any contractual breach does occur due to my participation in the said film/video, the said filmmaker and THIRD MILLENNIUM will not be held responsible. By signing this release, I have read and agreed to all of the above mentioned provisions.

RECORDING RELEASE FORM

In consideration of my choice to participate in Terrence J. Brady's Third Millennium feature production "Jumpcut," I hereby irrevocably grant to the said filmmaker the right to use the following # songs: "XXX" and "ZZZ" in the above mentioned video/film. I grant the said filmmaker to use the recordings in part or whole and allow the said filmmaker to edit the recordings at his discretion. I grant the said filmmaker to use any and/or all of the said recordings for any promotional purposes that may assist in the video/film's distribution. The said filmmaker claims no rights to any and/or all of the recordings and this release only secures the said filmmaker's right to use the recordings as mentioned here within. Any and/or all benefits from the use of said recordings [financial or otherwise] that are secured after the said video/film is completed, will belong exclusively to those who hold copyright.
